This presentation will be a personal story from Ben's time as a Marine Corps pilot. It affected Ben's leadership philosophy and has greatly influenced how he leads his company. If we choose to blame others instead of taking opportunities to learn, we will never maximize safety. Ben was directly involved in a mishap that left a young Marine with permanent injuries. Had the appropriate level of learning occurred after a nearly identical mishap a year prior, the second could have been avoided.
